    Paget's Disease

    What is this condition?

    Paget's disease is a slow-paced metabolic bone disease characterized by replacement of sound bone with excessive amounts of abnormal bone. The altered bone is fragile and weak, causing painful deformities of both outside shape and internal structure. Paget's disease usually settles in one or several areas of the skeleton (most frequently the lower torso), but occasionally affects many parts of the body.

    The disease can be fatal, especially when associated with congestive heart failure (widespread disease creates a continuous need for high cardiac output), bone cancer, or giant cell tumors.

    What causes it?

    Although the exact cause is unknown, one theory is that an early viral infection (possibly mumps) leaves behind a dormant skeletal infection that erupts many years later as Paget's disease. In the United States, Paget's disease affects approximately 2.5 million people over age 40 (mostly men). In 5% of cases, the involved bone experiences malignant changes .

    What are its symptoms?

    There may be no symptoms in early stages of the disease. But when pain does develop, it's usually severe and persistent and may limit the person's movement because new bone growth is rubbing on the spinal cord or sensory nerve root. The pain intensifies with exertion.

    The list of potential effects includes enlarged forehead, headaches, hunchback, barrel- shaped chest, and bowed legs and other changes in gait. The affected areas are warm and tender, susceptible to breaks, and slow to heal. The disease progresses to add bone growth that interferes with sight, hearing, and balance; complications such as hypertension, gout, and congestive heart failure may develop.

    How is it diagnosed?

    X-rays, taken even before symptoms appear, can show increased bone expansion and density. A bone scan, which is more sensitive than X-rays, clearly shows early so-called pagetic lesions. Bone biopsy reveals a characteristic mosaic pattern. Blood tests and other laboratory tests aid early diagnosis.

    How is it treated?

    Primary treatment consists of drug therapy and includes one of the following:

    • Cibacalcin, a hormone, given by injection, and Didronel to retard bone resorption (which relieves bone lesions). Although Cibacalcin requires long-term maintenance therapy, there is noticeable improvement after the first few weeks of treatment. Didronel produces improvement after 1 to 3 months.

    • Mithracin, an antibiotic that produces remission of symptoms within 2 weeks and biochemical improvement within 1 to 2 months. However, the drug may destroy blood cells or harm kidney function. Self-administration of Cibacalcin and Didronel helps people with Paget's disease lead near-normal lives. Still, they may need surgery to reduce or prevent fractures, correct secondary deformities, and relieve pinched nerves. Aspirin, lndocin, or Advil or Motrin usually controls pain.

    A Spray A Day May Keep Sinus Trouble Away


    Each year, nearly 37 million individuals in Dallas, Houston, elsewhere in Texas and around the country suffer from debilitating symptoms, including sinus pressure, nasal congestion, cough and postnasal drip that accompany sinusitis.

    Primary care providers often prescribe antibiotics to relieve acute sinusitis, which can develop following a chest cold. However, steroid nasal sprays -- either alone or with antibiotic therapy -- may ease symptoms and speed recovery better, as suggested in a new review by Israeli researchers.

    Sinusitis is an inflammation of the mucous membranes that line the sinus cavities. Steroid sprays like Flonase, Nasonex and Rhinocort work by reducing inflammation to promote drainage in the sinuses and are often prescribed to treat chronic sinusitis and allergy symptoms. But the use of steroids sprays for acute sinusitis is not as universally accepted.

    In this review, Anca Zalmanovici, a family physician at Rabin Medical Center in Petach Tikva, and her co-author, analyzed data from four randomized controlled trials including nearly 2,000 participants, all with clinical symptoms of acute sinusitis. Two of the studies evaluated patients at treatment centers in the United States, one took place in Turkey and the other included 71 medical centers in 14 countries.

    Study participants, who underwent X-rays or nasal endoscopy to confirm diagnosis, received either a placebo or intranasal corticosteroids for two or three weeks, alone or in combination with antibiotics. The intranasal corticosteroids used included fluticasone propionate (Flonase), mometasone furoate (Nasonex) and budesonide (Rhinocort).

    Overall, 73 percent of the patients treated with nasal steroids experienced relief or marked improvement of symptoms during the study period, compared with only 66.4 percent of patients who received the placebo.

    “For every 100 patients treated with intranasal corticosteroids, seven additional patients had complete or marked symptom relief," compared to those in the placebo group, the reviewers found.

    Researchers pooled data from three of the four studies, excluding the lowest-quality study from the statistical analysis. None of the studies reported serious side effects, and rates of sinusitis relapse were similar between the treatment and placebo groups.

    Stronger doses of nasal steroids appeared to work better. Patients receiving daily doses of 400 micrograms were more likely to experience relief of sinusitis symptoms, than were patients receiving 200-microgram doses.

    Although there is not enough evidence to suggest that nasal steroids can stand alone for acute sinusitis treatment, “the results of these studies and reviews support the current clinical rationale of adding an intranasal corticosteroid to antibiotic therapy," reviewers said. Allen Seiden, M.D., director of the University of Cincinnati Taste and Smell Center, said that more data are required

    before routine recommendations on intranasal corticosteroids can be made. “It seems to have been a well-conducted review, with thorough statistical analysis. However, in the end, it analyzed relatively few studies," Dr. Seiden said.

    He added that the review lacked information about how individual diagnoses were made, and said that even with X-rays and nasal endoscopy, distinguishing between viral and bacterial infections can be difficult, a problem that may influence the choice of treatment.

    When it comes to treating sinus infections, “patients vary as to when they will seek medical intervention. Some will come in after only a day or two of symptoms; some not for two to three weeks," Seiden said. Longer waits can make symptoms harder to treat, he said, “while many patients with symptoms for only a few days will in fact have a viral infection."

    Although there are few downsides to using nasal steroids such as those in the review, they do tend to be fairly expensive. According to the National Institute of Allergy and Infectious Diseases, diagnosing and treating sinusitis costs Americans nearly $6 billion every year.

    Retinoblastoma is a cancer of the retina. Retinoblastoma is the most common primary ocular malignancy of childhood. Although this disorder can occur at any age, it usually develops in young children. Most cases of retinoblastoma occur in only one eye, but both eyes can be affected. Untreated, retinoblastoma is almost always fatal, hence the importance of early diagnosis and treatment.

    Retinoblastoma can occur in two forms:

    An inherited form where there are often tumors in both eyes (bilateral) or sometimes only in one eye.

    A non-inherited form where there is a tumor in only one eye (unilateral)

    Retinoblastoma is related to chromosome 13. Retinoblastoma occurs when a cell of the growing retina develops a mutation in the RB gene (a tumor-suppressor gene). This mutation causes the cell to grow out of control and become cancerous.

    Mutations in the RB1 gene are inherited in an autosomal dominant pattern, which means that one copy of the altered gene in each cell is sufficient to increase cancer risk. A person with retinoblastoma may inherit an altered copy of the gene from one parent, or the altered gene may be the result of a new mutation.
